At Ōtautahi Clinic we collect health information in various ways. This includes but is not limited to:

Information we commonly collect include:

The information collected will be used to provide healthcare for you and is not utilised for other purposes. Only staff who need to see your personal information will access it.
In general, we will send a referral letter back to your GP but we will otherwise not share your health information with others unless this is required to comply with legislative obligations such as a court order, or to prevent serious harm to yourself or others.
Information collected is stored on an electronic, cloud-based patient management system called “Elixir” which complies with New Zealand standards for security. Some clinicians may also use paper records which will be kept securely.
Ōtautahi Clinic also utilises “Heidi”, which is an ambient AI tool that records the consultation and then summarises it. It does not keep audio recordings of consultations and identifiable information is removed prior to processing in a secure database.
You have the right to request access to and correct your personal information. Please request this in writing and our practice will respond as soon as possible.
If you are concerned that your right to privacy has been breached please contact the clinic and we will do our best to help. If we are not able to address your concerns adequately then you can also contact the Privacy Commissioner.
